Student Permit Requirement?

Midwife Apprentice license required. To obtain the license the apprentice must provide: verification of apprenticeship arrangement, proof of enrollment in midwifery education or an approved course of study, evidence of negative TB testing, and $50. The license is valid for one year and renewals must be submitted at least 90 days prior to the expiration of the initial license. The Midwife Apprentice License can be renewed three times before taking the NARM. Extenuating circumstances can allow for a fourth renewal at the discretion of the Department on a case-by-case basis.
